Mommy and Me
Overview
“Mommy and Me” program provides a comprehensive suite of services designed to improve maternal health outcomes tailored to support underserved pregnant women and new mothers through case management and care coordination. The program will offer individualized one-on-one support and community-based education to encourage healthy behaviors and care practices throughout the perinatal period. This program is designed to serve pregnant and post-partum women who reside in Angelina, Angelina, Cherokee, and Nacogdoches Counties.
Services Include:
- Free Transportation to Medical and WIC appointments
- Virtual and In-Home Case Management/Care Coordination<
- Resource Navigation – Robust Referral Network
- Group-Based Health and Parent Education
- Community Voice Incorporated via Community Consortium Member Opportunities
- Provide Client Needs Support (Pregnancy and Baby-Related items)
Eligibility
Individuals eligible for Mommy and Me services are expecting moms and their partners.
